As many of you know, NCHSAA announced that the dead period has been lifted as of June 15th (today). Sports will return in phases following state and local guidelines. If you are a student-athlete who plans to play sports at Lumberton Senior High School then there are forms that your parent/guardian and physician need to complete and sign.

Each student-athlete is required to fill out the NCHSAA Initial Screening Questions form and it be signed by both student-athlete and a parent/guardian. Click here to view, download and print the NCHSAA COVID-19 Initial Screening Questions form. .
If the student-athlete has had symptoms of COVID-19, but the test results were negative, he or she will need a licensed healthcare provider to fill out and sign the NCHSAA Return to Play form. Click here to view, download and print the NCHSAA COVID-19 Return to Play form. . This form also requires the signature of a parent/guardian.
If the student-athlete was diagnosed with COVID-19, a licensed healthcare provider must fill out and sign the NCHSAA Return to Play form for a student-athlete who was diagnosed but is now clear. Click here to view, download and print this form. . This form must also be signed by a parent/guardian.
